Coaching Agreement Between Autonomic Coaching and ______ Coaching Agreement - Client Contract This agreement is entered into by and between Autonomic Coaching and _ whose address is _______________________________ referred to as the “Client” where Autonomic Coaching, referred to as the “Coach” agrees to provide Coaching Services for the “Client” focusing on the following [methodology/outcomes/goals]. Description of Coaching: Coaching is partnership, defined as an alliance, not a legal business partnership, between the “Coach” and the “Client” in a systematic process that assists the “Client” to take charge of their health and vitality by facilitating the creation of a scientific formula refined and tailored to their personal chemistry and biology and carry out a strategy for achieving those goals. • Improvement on systems dysfunction based on participation of protocol derived through the outcome of molecular diagnostics • Tangible weight loss changes based on efficiency in the participation of protocol application • Improved vitality based on participation of protocol application • Increased cognitive function based on the outcome of protocol application • Emotional integrity based on participation of all factors of coaching and protocol application • Improved digestion based on participation in the application of the protocol Coach-Client Relationship a) “Coach” agrees to maintain the ethics and standards of behaviour established by the International Coach Federation (ICF). It is recommended that the “Client” review the ICF Code of Ethics and the applicable standards of behaviour. b) “Client” is solely responsible for creating and implementing his/her own physical, mental and emotional well-being, decisions, choices, actions and results arising out of or resulting from the coaching relationship and his/her coaching calls and interactions with the “Coach”. As such, the “Client” agrees that the “Coach” is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any services provided by the “Coach”. “Client” understands coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure or treat any physical, mental or medical diagnosis. c) “Client” acknowledges that coaching is a comprehensive process and requires dedication and commitment. © Copyright Envision Corporation. 2002. Autonomic Coaching Agreement Page 2 of 5 d) The “Client” understands that in order to enhance the coaching relationship, the “Client” agrees to communicate honestly and be open to feedback and assistance and to create the time and energy participate fully in the program. Services a) The parties agree to engage in a coaching program through for a duration of 6 months, commencing on 1 Jan 2021. b) The method of coaching will be conducted via WhatsApp during the hours of 8 AM- 9 AM GMT and 2MP – 4 PM GMT c) The “Coach” will be available to the “Client” by e-mail and scheduled meetings on Mondays with Marinus Van Standwyk and on Fridays with Justin Maguire as booked through their various calendly links. Additionally Fiona Black would be available based on Calendly schedule to consult with the “Client” on alternate weeks for 30min intervals. The responsibility to book these calls is solely to that of the “Client” provided for the “Coach” enables route of calendar availability. d) The time of the coaching meetings has a duration of 15 minutes per session for Marinus Van Standwyk and Justin Maguire and 30min for Fiona Black . This is a virtual meeting. e) Review of Molecular diagnostics is done on receipt of evaluations done to labs received, this is provided for based on the tier of service agreed to. On review the “Client” will have 45min review in which to run over the findings with both the “Coach” and a medical consultant, availability of these calls will be made on Saturdays and in special circumstance Tuesdays. Note medical consultation only takes place based on the outcome of findings from blood chemistry analysis, not urinary, stool, saliva, dry blood spot nor hair follicle test (functional tests). Additional ‘functional’ tests will be reviewed with client for 30min based on client scheduling this call on Wednesdays through provided Calendly link, this call will be delivered by Justin Maguire. Schedule and Fees A Sign up fee of is payable as well as a monthly payment of ___ per month on or before the 1st of the month for _ months Confidentiality This coaching relationship, as well as all information (documented or verbal) that the “Coach” shares with the “Client” as part of this coaching relationship, is bound by the principles of confidentiality and may not be copied, distributed nor published unless otherwise permitted to do so by the “Client”. The “Coach” agrees not to disclose the “Client’s” name as a reference or any information pertaining to the “Client” without the “Client’s” written consent. Confidential information does include information that: a) was in the “Client’s” possession prior to the commencement of this agreement. b) is generally known to the public or in the “Coach’s” industry. © Copyright Envision Corporation. 2002. Autonomic Coaching Agreement Page 3 of 5 c) is obtained by the “Client” from a third party, without breach of any obligation to the “Coach”, d) involves illegal activity Release of Information The “Coach” engages in training and continuous education and or maintaining required certifications and credentials. That process requires the names and contact information of all “Clients” for possible verification. By signing this agreement, you agree to have only your name, contact information and start and end dates of coaching shared with relevant Boards. According to the ethics of our profession, topics may be anonymously and hypothetically shared for training, supervision, mentoring, evaluation and for the professional development of the “Coach” and for consultation purposes. Record Retention Policy The “Client” acknowledges that the “Coach” has disclosed his/her record retention policy with respect to documents, information and data acquired or shared during the term of the “Coach-Client” relationship. Such records will be maintained by the Coach in a format of the Coach’s choice (print or digital/electronic) for a period of not less than 5 years. Limited Liability Except as expressly provided in this Agreement, the “Coach” makes no guarantees, representations or warranties of any kind or nature, express or implied with respect to the coaching services agreed upon or rendered. In no event shall the “Coach” be liable to the “Client” for any indirect, consequential or special damages. Entire Agreement This document reflects the entire agreement between the “Coach” and the “Client” and reflects a complete understanding of the parties with respect to the subject matter. This Agreement supersedes all prior written and oral representations. The Agreement may not be amended, altered or supplemented except in writing signed by both the “Coach” and the “Client”. Exit Clause The client herby agrees to pay a 50% on the remainder of the contract should he/she wish to use the exit clause. Example: Should client x signs up for Tier one and 3 months in decide to use the exit clause, the remaining 3 months will be payable at a 50% fee.. Nutraceuticals All nutraceutical orders will have a waiting period of 28 days. Nutraceuticals paid for and sent via DHL is at the risk of the client, Autonomic Coaching does not take responsibility for the impounding of nutraceuticals and or medication by the customs of your country. Should you wish to pay additional insurance please let us know. Autonomic Coaching Agreement Page 4 of 5 Services rendered to the client The following services are rendered for the moneys paid, the services form part of the Autonomic Coaching option that was discussed. · Health protocols design every three months · Laboratory diagnostics (based on tier selection) every three months dependant on the coaching option purchased. · Exercise programming · Movement screen analysis · Dietary programming · Access to questions you may have via WhatsApp during the times stipulated under services. · Access to calls with your coaches twice per week for 15 minutes booked through Calendly · Access to mindset calls twice per month for 30min booked through Calendly · Access to your medical consultant every 3 months based on outcome of blood chemistry review What this package does not include: · Additional function medicine testing · Nutraceuticals required · Medication required · Monitoring equipment (blood glucose barometer, thermometer, pulse reader, body composition scale/monitoring device) · Exercise equipment or gym membership · Additional time requested with either mindset coach or medical consultant · Additional call time requested outside of allotted within coaching agreement Binding Effect This Agreement shall be binding upon the parties hereto and their respective successors and permissible assigns.
